What Does Rehabilitation After Limb Amputation Involve?

From a former human amputee physiotherapist's perspective
One of the questions I get asked most often is:
"My pet has just had an amputation. What happens next?"
Before becoming a small animal physiotherapist, I worked with human amputees in the hospital's amputee clinic. After a lower limb amputation, not only does walking become more physically demanding, but patients also face significant psychological challenges. Supporting them through rehabilitation taught me that recovery after limb loss is about far more than simply learning to walk again. It's about adapting to life after limb loss while rebuilding strength, confidence and independence.
Moving into small animal rehabilitation, I found that while many of the principles of rehabilitation remain the same, four-legged movement brings its own unique challenges. Unlike humans, dogs and cats rely on both their forelimbs (the equivalent of our arms) and hind limbs to support their body weight. Losing either a front or back leg therefore changes how they balance, move and distribute forces throughout the rest of their body.
Dogs and cats are incredibly adaptable, but adapting doesn't always mean moving efficiently. Rehabilitation plays an important role in helping them move more efficiently, build strength and reduce unnecessary strain on their remaining limbs.
What is an amputee?
An amputee is an individual who has had part or all of a limb removed.
Dogs and cats may require an amputation due to:
Severe trauma (e.g. road traffic accidents or severe fractures)
Cancer (such as osteosarcoma)
Severe infections that cannot be managed
Loss of blood supply (ischaemic necrosis)
Congenital limb deformities
Paralysis or a painful, non-functional limb
Severe arthritis where other treatment options are no longer suitable
Although the decision is never easy, amputation is often performed to remove a painful or diseased limb and improve their comfort and quality of life.
How does losing a limb affect my pet?
Just like humans, dogs and cats have to work harder after losing a limb. Work that was previously shared between four limbs must now be performed by three (or less).
In humans, walking after a lower limb amputation requires more energy. The increase varies depending on the level of amputation, walking speed and whether a prosthesis is used, but the demands are generally greater following an above-knee amputation, with studies reporting energy expenditure increasing by up to approximately 65%.
There are currently no similarly established energy expenditure figures for dogs or cats. Instead, most veterinary studies have examined how dogs redistribute their weight and forces across their remaining limbs.
How a pet adapts also depends on whether a forelimb or hind limb has been removed.
Forelimb amputation
In dogs, approximately 60% of body weight is normally carried through the forelimbs. As a result, forelimb amputees often face greater biomechanical demands than hind limb amputees.

These are normal adaptations that help dogs stay mobile, but they also increase the workload placed on the remaining joints and muscles, potentially increasing the risk of overuse injuries such as carpal hyperextension and shoulder overload.
Hind limb amputation
Dogs with a hind limb amputation generally adapt more easily than forelimb amputees, but they often have more difficulty accelerating and producing propulsion.

Over time, these increased demands may contribute to repetitive strain on structures such as the hock, Achilles (calcaneal) tendon, hip and knee.
One of the most important things to understand is that not every compensation is a bad compensation. Some changes are necessary for efficient movement. One of the roles of a physiotherapist is recognising which adaptations help your pet move well and which may eventually contribute to pain or injury.
The aim is not to make your pet move exactly as they did before. It is to help them compensate as safely and efficiently as possible.
Why is rehabilitation important?
Many dogs and cats surprise their owners by standing or walking within a few days of surgery.
However, being able to walk is only the beginning of recovery.
Rehabilitation aims to help your pet:
Manage pain
Regain confidence with movement
Improve balance and strength
Build core and proximal muscle strength
Reduce unnecessary strain on the remaining limbs
Improve walking endurance
Return to daily activities safely
Rehabilitation also supports owners by helping you understand what your pet is experiencing, what changes are normal, and how to support them throughout recovery.
What should I expect after surgery?


The first two weeks after surgery are often the most important. The main priorities during this period are:
1. Pain management
Amputation is a major surgery, and pain control is essential.
Your veterinarian will prescribe appropriate pain medication. Depending on your pet's condition, rehabilitation may also include cold therapy, TENS, laser therapy, PEMF, taping or gentle massage.
Some animals may also experience phantom limb sensations, where the brain continues to perceive sensations from the missing limb. Although this is difficult to confirm in animals, helping your pet become comfortable with gentle touch around the surgical site may help with desensitisation.
2. Wound healing
Follow your surgeon's instructions closely. This may include:
Keeping the wound clean and dry
Preventing licking or chewing
Monitoring for redness, swelling or discharge
Attending scheduled postoperative reviews
Rehabilitation input may begin during this early period through pain management, positioning, safe mobility and owner education. More active rehabilitation exercises can begin once the surgical site is stable and your surgeon has provided clearance.
3. Preventing pressure sores
After surgery, pets often spend more time resting and may favour one side.
Check the skin daily over bony areas such as the shoulder, pelvis, hip, knee and hock. Soft bedding and gentle changes in position can help reduce prolonged pressure.
What does an amputee's rehabilitation journey look like?
Once your surgeon confirms the wound has healed well, rehabilitation can gradually progress. This is typically after the stitches have been removed.
Depending on your pet's needs, it may include:
Gentle, progressive strengthening
Balance and weight-shifting exercises
Core and proximal muscle strengthening
Maintaining joint range of motion in the remaining joints
Cardiovascular conditioning
Desensitisation of the stump
Gait and functional movement training
Education on expected compensations and movement patterns while monitoring for early signs of overloading
Strengthening the core and other muscles close to the trunk is especially important, as they provide a stable base for movement and help support the remaining limbs.
The rehabilitation plan should also consider why the amputation was performed. A pet recovering from trauma may have different needs and goals from one receiving ongoing treatment for cancer.
What about orthotics and prosthetics?
Not every amputee requires a prosthesis.
For some pets, particularly those with partial limb amputations, a custom orthosis (splint) may help support the remaining limb and reduce excessive strain.
A prosthesis may be appropriate for selected patients depending on factors such as:
The level of amputation
The condition of the residual limb
Joint mobility
The pet's temperament and tolerance of handling
Lifestyle and activity goals
The owner's ability to manage the device
If a prosthesis is being considered, rehabilitation may also include preparing the residual limb for weight-bearing and maintaining muscle mass before fitting.
After fitting, pets are gradually introduced to the prosthesis, progressing from tolerating the device to standing, weight shifting and eventually walking.

One of the best ways to protect the remaining limbs
Maintaining a healthy body condition is one of the most important ways to protect the remaining limbs.
Even a small amount of excess weight increases the workload placed on the remaining limbs. Body weight, body condition and muscle mass should therefore be monitored throughout recovery and long-term management.
Simple home adjustments can also help:
Provide non-slip flooring
Limit uncontrolled stair use
Use ramps where appropriate
Provide supportive bedding
Reduce unnecessary jumping

Rehabilitation isn't about teaching your pet how to move on three legs — they're often incredibly good at figuring that out themselves.
Instead, rehabilitation helps them move as comfortably and efficiently as possible while reducing unnecessary strain on the rest of their body throughout recovery.
With the right guidance, realistic expectations and a tailored rehabilitation plan, your pet can continue to enjoy a wonderful quality of life after limb loss.
